• Health and Nutrition Data Collection
• Assist in community health assessments to identify priority health and nutrition needs.
• Identify individuals requiring medical or nutrition services and facilitate referrals to appropriate health facilities.
• Follow up with referred cases when required to ensure access to services.

المؤهلات المطلوبة

• Diploma degree in Public Health, Community Health, Social Sciences, or a related field.
• Minimum 1 yr of experience in community-based health or nutrition programming.
• Familiarity with community health education methods and behavior change communication approaches
